Fixing Shoulder Pain: Harvard Scientists Develop a Method To Restore Damaged Tendons and Muscles

The new complex tissue platform can restore damaged rotator cuffs

The typical office worker often has soreness throughout their body as a result of their sedentary desk jobs. Even young individuals may develop shoulder pain, which was previously primarily an issue for elderly people. Once shoulder pain creeps in, it is difficult to dress oneself, let alone move one’s body freely. It is also difficult to fall asleep. While the rotator cuffs are often naturally harmed as we age, repairing them has shown to be difficult.

Through a collaboration with Professor Hak Soo Choi at the Harvard Medical School, a Pohang University of Science and Technology (POSTECH) research team made up of Professor Dong-Woo Cho, Dr. Suhun Chae, and Jinah Jang, as well as Professor Jinah Jang and Ph.D. candidate Uijung Yong, has developed a complex tissue platform that can repair damaged rotator cuffs. This platform, which can precisely replicate the intricate structure of rotator cuffs, is 3D-bioprinted using tissue-specific extracellular matrix bioink.

The international journal Bioactive Materials recently published the findings of this study, which could potentially provide patients with chronic shoulder pain renewed hope.

The study team transplanted this platform in rats that had full-thickness rotator cuff injuries. The researchers observed tissue regeneration and recovery in shoulder function. The results proved that the platform, which includes stem cells, can actually regenerate rotator cuffs.

Notably, the researchers combined tissue-specific bioimaging agents together with near-infrared fluorescence imaging to visualize this process. The researchers were able to use this technique to monitor anatomic change and regeneration processes in the animal model in real time and in a non-invasive fashion.

This platform offers a microenvironment and components similar to those of the actual tissue. Therefore, once applied to patients, it is expected to have high treatment benefits and eventual recovery of shoulder function. It is particularly beneficial for those patients who cannot use autologous tissues to regenerate rotator cuffs by providing a customized treatment option.

    I have suffered with a massive torn rotator cuff. Two additional attempts to repair the tear have been unsuccessful. The right rotator cuff is also completely torn and five surgeries have been unsuccessful. Would you be interested in performing a test surgery and see if one or two of the rotator cuffs could be repaired. Please advise.

  5. I had rotator cuff repair last Nov. 2021. After extensive physical therapy for 6 months, I can raise my arm but could not reach back to fasten my bra or let alone wipe myself using the right hand. I would like to be considered for your advanced treatment, if possible.

    • If in Florida area get with a John Mehalk in Fort Myers.
      He did my rt shoulder after 1 other messed it up, and I had a Doctor tell me he could fix me after bad operation. Once they got my rt arm to move all around this Doctor told me he was not qualified to FIX me. Will not say the words I told him. He then recommened John Mehalk. I went to him then and 1st time I seened him he put a needle into top right arm, and said in 15 minutes he would be back in room. He came back and took my arm and went in every derection with it and NO PAIN. He ask me when do you want a operation. YES he is the Whiss kid at the time. Like I said NO ONE would touch me, but he was young and knew how to repair me. Good luck.

  9. I have had 3 rotator cuff surgeries and would love to be considered for surgery to repair rotator cuff. I had two surgeries on the right cuff and 1 on the left cuff the right give me the worst pain,with sleepless nights.

  10. I’m only in my mid 40s, but my dominant hand shoulder started giving me trouble so I can even touch my alternate armpit without pain. Something as simple as swatting flies causes pain to radiate down my upper arms. I’m scared. Hopefully it’s nothing serious. Bless this new procedure if it works!

  14. I had RC repair surgery on my right shoulder 17 years ago, and I am happy that I have mobility in it, but I often get shocking pain with certain movements.       Hello 46 years of age. I am a painter by trade and a all-around handyman. My body stay in 12 to 15 hours a day 6 days week. There is days when I hurt so bad I don’t think I will make it through. But with a wife a 1 year old baby girl and a 3 year old all boy all energy . And now a new born boy. Depending on me I can’t stop or slow down COVID took a big chunk of work. And the stress piled up. Still trying to get back to profit. Not easy taking on more jobs then should. Coming home to a 3 year old daddy’s guy that doesn’t understand that I am hurt and am tired. So what I am rambling on about if there is human studying and need volunteers.   23. While this is an awesome development I very highly doubt anyone outside of the 1%ers could ever afford such a procedure. Furthermore, I really think that all the great minds should be focused on immediate ways to help reverse climate change or no one’s even gonna be around by the time this procedure is available for testing on humans.

      I have lost the use of both my arms. Due to rotary cuff and tendon injury. Doctors have told me I qualify for reverse shoulder replacement surgery. But due to the extent of the damage I will never have full use of either arm. Surgery would only help with the pain. Help would be forever greatful

    Platelet rich plasma (PRP) and stem cells taken from your hip have been successful in treating rotary cuff tears. There are several clinics in the US that use PRP, your harvested stem cells and ultrasound images to inject your injured shoulder. Physical therapy is critical to success. Google orthobiologics for more information. The new technique described in this article looks very promising.

    It appears you have chosen a brilliant place to begin your regeneration procedure. You already have a long line of volunteer patients.
    I too have suffered through rotator cuff injury, surgery and recovery only to re-injured my right rotator a year ago in a rafting accident.
    If you are considering possible human rats for for experiments, please consider my case as well.

  82. I’m 51 but don’t look it. I’ve been a glazer since I was 20. I use to work out lift at the gym and home 5 days a week. I had to stop at 42. I’ve had rotator surgery once. I’ve needed it again but 5 years ago was force out of the company that I started alone I n 93 and then had to watch them destroy it in 18 months. I’ve had to live with the daily nightly pain in order to support my family. Around 6.months ago I was going down a grass hill side and slipped. Something happened.to my left shoulder.     I tore all 4 rotator coff tendons in 2005. Surgery could only repair 3 tendons but I have good ROM.I also had stem cells injected in both shoulders with good results. I canbstill paddle and surf at 71. Stem cell therapy with traditional surgical repair has worked and might be much more affordable.

      First, congratulations to the POSTEC team and Prof. Hak Soo Choi of Harvard for their breakthrough development for shoulder repair!
      I am a 62 year old woman with mobility issues from a back operation and a fall. I also have back/ hope arthritis. As for the shoulder I have a nagging constant low grade pain in my right shoulder. I have fibromyalgia which pains the soft tissue areas of my shoulders… 8 years ago I was told I have bursitis in my right shoulder. But to this day I haven’t been given any imaging of the shoulder. It’s been getting worse to the point I can’t reach back/behind to grab something. If I forget I’m in agony. Sometimes I feel like my right hand is getting weak. My doctor is aware and hasn’t done anything. I’m tired of pain. I don’t take painkillers because I don’t want to damage my liver… I have other pain issues in other areas of my body… Anyway I was hoping you could advise and maybe use me as a study candidate.

  91. I had serious shoulder arm and hand issues. I was told it was tendonitis and tears in my rotator cuff. I had a Platelet Rich Plasma shot in two areas of my shoulder in one visit and I nearly have full range of motion in all areas of my hand and arm. I would try a PRP shot before any of these new developments, its easy, inexpensive, proven, and uses my own blood.

You can fix the cuff all you want but if the muscle attached the tendon is already atrophied it cannot come back so until they find a way to regenerate muscle tissue this will not work for people with chronic tears there have been many tissue scaffolds attempted over the years and they’ve all been uniformly marginally successful only. Studies have shown any repair tendons the muscle does not reverse the fatty atrophy.

    I took care of my wife foe 5vyearsvwho had Alzheimer and no use of arms and legs. While moving her from bed to wheelchair she stumbled and I caught her before she hit the floor. However. I pulled my left shoulder out of the socket. I live with the pain, but would be a volunteer. I am 83 in excellent health.

Hi, I would like more information about this my husband was in a really bad vehicle accident which should have taken his life but God wasn’t finished with him yet, however we don’t have medical insurance and have been on going with lawyers ECT and doctors doesn’t want to perform until they get paid up front of course. However once our lawsuit goes through and finish they would be paid by the 18-Wheelee company to whom which was at fault since the accident my husband has been in excruciating pain, not only has that specific surgery got put on hold but all other surgeries as well the accident didn’t just physically get my husband, but physically, mentally, emotionally to himself, but also me as his wife and the mother of our children have been through it all as well and his children, there will never be a curtain dollar amount that will ever fix everything that has been broken from this accident but of course they will try and settle, but unless it’s well worth what we all have been through already not to mention still to come the road ahead isn’t closed so we will still go through much more daily until the end of time.     Ive sustained multiple dislocations and have delt with the pain of my shoulder pain for about half my life now… i would LOVE to.be considered for this treatment. I hope it would even help as I have undergone no surgery and would hope that because of that it could give you even a better insight on the repair of the cells and tissue that are damaged and being repaired by the treatment alone.

    I suffered serious shoulder injury w several tears including rotator cuff as well as severe instability.
    The diagnosis of several physicians per repeated MRI imaging over several years dictated complete reconstruction.
    As I was undergoing many procedures for multiple injuries over time while dealing w insurance coverage issues the injury was obliged to linger.
    Settled for injections and PT for a 3 year stretch since previous MRI.
    At one point it became unbelievably painful at night, where I would sit up to the sound of screaming to realize it was me making the sound.
    It calmed down by the time pre surgical imaging was done.
    Lo and behold the shoulder no longer showed any tears or injury and though sore was totally functional no longer unstable ( previously it easily could slip completely out).
    Upon being told results I asked my Doctor how this was possible as I had understood such tendon injury didn’t regenerate spontaneously.
    As he looked at the films in a bit of amazement he replied ” really weird scar tissue maybe ?”
    The imaging was repeated with same results.
    I haven’t had trouble with the shoulder at all in many years since.
    Never really tried to figure out the mystery as life got in the way.
    Always curious though.

  108. Sounds to good to be true; so how long will it take for you to start trying it out on humans. While it working on a rodent is one thing, will it be feasible to try on human beings. There’s a process you have to go through in order to get it approved for trial first before you can actually start performing this procedure on anyone. Saying that how long will it take you to do this or have you tried already or in the process stage now??? I would like to know as well as the other’s. No one wants to keep living in pain 24/7,especially if this is approved and will be available to the public. Also what is the price tag for this procedure going to be,and where will it be available if it’s approved.     I am an orthopedic surgeon with a complete rotator tear. I had my nurse inject PRP into the capsule and also found that pain was coming from the deltoid muscle. That was injected with dexamethasone and marcaine. I got 80% relief and can sleep on the shoulder as well. I sympathize with all of you..below is my office site…be glad to offer a consultation. Surgery is not the answer, but it is important to figure out the pain generator.

    I live alone off grid at 4,700ft on Mauna Loa.

    At nights always in the 30’s

    I also have been building by myself. A 4 room art studio with a Rock walled den and and fire place..and a separate kitchen n cottage that was for after my operation.
    But I think I’m going to change my mind, especially after reading how it doesn’t actually work. This new method seems promising. After all the therapy n preparations Doctors won’t operate on me, with no one to watch me. If I get the operation, then I can’t hunt
    That’s how the dogs n I eat.
    The reason I put all of this down? Not all of us can endure pain, I just have to bite the bullet

I was fortunate to have a complete full thickness tear of my left infraspinatus and supraspinatus tendons repaired laparoscopically 3 years ago and did all required post surgery therapy and then some. I continue shoulder therapy still today. My shoulder feels better than in my 30’s and with about 98% range of motion. Very very happy. I know a lot of shoulder injury patients don’t have these results so I can see how promising this news sounds to many active people out there.

  141. Please don’t publish or share any of my info. including my name. In January of 2019 I slipped on ice covered asphalt at work. Witness said I was flat in the air and slammed down horizontally hard.I came away with many injuries and severe pain that I never experienced in my active life. Amongst the injuries my dr.s and their dr.s( workmen’s comp.) deemed the worst were to my spine (cervical and lumbar) and my shoulders. I couldn’t sleep naturally because of the crazy severe pain. I would sit and or lay down until I fell asleep from exhaustion only to be bolted out of sleep by lighting pain! Back to the shoulders the dr.s both sets said I had two “massively rip led rotator cuffs”. Both were very painful with the right being the most painful. To quote dr.s notes both shoulders the humerus in were both migrated from the glenoid. Two shots of cortisone in the left and three shots in the right did nothing for their pain. In a try for pain relief my dr. did arthroscopic surgery any scare tissue, debris etc. found out later he also removed some atrophied ligaments and detached one of my biceps. Leaving my right arm pretty useless and still in pain. Everything more complicated than what I detailed here. Please contact me if you think I could be a candidate for your procedure so I could give you all the details. As it stands my dr. says The best he can do is a reverse shoulder rebuild giving me about 63%of the movement I once had. Two years ago I contacted a stem procedure clinic (Gio Star) ask if the could grow back my ligaments and my tendons / bicep muscle thy said yes but with no guarantees?! So I I could go back to my dr. and fix my rotator cuff in a normal way! He said not gonna happen. To a head of the tech/ surgical times. Came across this article and was filled with hope again! That was even after extensive pre surgical pt, and immediate pt following surgeries, and every day working the knee since 2016.     I had surgery 7 years ago on my left shoulder, I had 2 torn ligaments in my rotator cuff. The surgeon said it would be a three hour surgery. However, during the surgery it was explained to me that my bicep had been detached in the same area which extended the surgery to 5 hours. I have been playing bass guitar since high school, except now my fingers get tingly and numb after playing a few songs, also when riding my motorcycle my left hand gets numb as well hold the throttle. I say this because now my right shoulder has a torn rotator cuff. I’ve known this for 3 years but reluctant to have surgery. I’m afraid the same thing will happen. I would like to volunteer if the option is still available.

    And xrays look like 2 rotten apples
    This due to work related and age
    Would love to hear more or actually receive this treatment.
    Have an 11 yr.old son with special needs and a 15 yr old daughter.
    So Retiring at 56 is not an option.

    Left should has lots of scar tissue from an injury suffered in an accident in 1982 which leaves me with very limited range of motion. This lead to overuse of my right shoulder and an irreparable rotator cuff.

    I would very much appreciate consideration for this procedure. I fell on my left arm which caused limited movement to my rotator cuff and associated muscles. It was an accident and not my fault and recovery has been slow. Reverse should replacement was recommended but did want to go to that level of operation so have been exercising upper extremities which has helped but still not fully recovered. Would appreciate the opportunity to be back to somewhat normalacy. Thank you.

    I was run over while asleep in my bed and broke my shoulder. Rotator cuff surgery and therapy took care of it but lost range in motion, strength and constant but tolerable pain.
    Would love to play catch with my boys as they get older. No pain will be a plus.

  252. I encourage anyone with rotator cuff pain, or other musculoskeletal or neurological pain or limitation to look into Feldenkrais (movement re-education) lessons. Gentle and non-invasive. Classes are available on Zoom. Typically $15/class. Been keeping me out of a wheelchair for 2 decades. Worth giving it a try to see if it helps. Google for the Feldenkrais Guild for more info and sample lessons.

    I’m suffering from deterioration of my left shoulder. I’m 84 years old and was an avid golfer but now unable to do anything. The orthopedist won’t do surgery because of my age.     Had a 3.5inch tear right shoulder dr.said bad tendent damaged bla bla bla short version I went home and slept on the options have thatdoc fix or just put it back together my self so next day called doc. Told her I’m thankful but I’ll pass..And remind you could not get my arm up pass my wast.at 6ft.i have began to get in touch with myself and practice self heeling witch I’ve always been a believer in the mind so I made my self work the feeling of pain into one spot and places askin burning kettle on cloth and start by seeing beauty in pain into my arm I focus on pushin the pain closer to the skin and keep it together after three day of burning my self in program myself and practice self heeling I don’t have one pain I don’t know what I did but possibly self heeling is possible…💪I don’t recommend it to anybody else just let your self as postive as u are ur beauty will not help but ur instead ur mind and sole will take care of it….

Excellent results,but they want to replace the ball joint on my left and I won’t let them because I know it can be healed. Please advise me. Total disability and over 65. Good health thanks

    This is very exciting news and I would love to be a volunteer; been with right rotator cuff tear for over a year and steroid shot, multiple PT courses-still doing at home.. my life has changed! Unable to have a normal life, no proper sleep etc…My shoulder specialist Dr. (Who is super)said surgically not fixable with current technology..
    I’m 68 years old, otherwise pretty good health.

    8 months ago I completely tore my Longhead bicep muscle which now lays at the bottom of my forearm. I don’t know how I’m still as strong as I am. My concern is I was also told my rotator cuff and AC muscle are torn/tearing. I’m a 55 female in excellent shape and i have to keep working doing landscaping to support myself. I’m not in extreme pain, just discomfort and shoulder gets “caught” and pops frequently. I’m affraid if my Rotator cuff n AC isn’t fixed I’ll do more damage and loose my ability to work. Ide like any information or treatment available. Thank you.

    I have had 2 rotator cuff repairs on the same shoulder. Following the 2nd surgery I had good results following PT and healing. Over time and use the shoulder injury is persistent and I have chronic pain in the shoulder. I would absolutely be interested in volunteering as a test case.

  272. I had non invasive 4 point surgery on my right shoulder 10 or so years ago have developed very bad neck and shoulder pain since then, I think they are related the shoulder. I had a test done a # of years back after orginal surgery, ortho said that it had a slight tear in it, it has gotten worse since then I am still able to lift arm and rotate it but have pain in doing so .That being said I try not to lift much as it makes it much worse. I know that if I do not do something it will continue to deteriorate.

    I had a motorcycle accident when I was 10 years old. I lost my right shoulder arm and wrist from muscle nerves legments and tendon damage. I have no muscle in my shoulder with two surgeries. I had 8 pins and metal plate put in at 12 and was taking out at 14. I have had 3 surgeries on my hand and wrist to help me use my fingers again. Tendon transfers and legments lengthen. I am 44 now and I still have no control over my right shoulder arm wrist. I have no feeling for the most part and what I can feel it’s in areas that u are not touching. I would love the chance to feel normal and ppl look at me normal again..

  278. I would love so much to have my shoulder fixed. After the surgery I was told that it was much worse than they thought it was and that I need to have a shoulder replacement but I am too young to get it don’t because I would need it replaced in about 15 years. Sucks being old but not old enough. I am sure having Ehlers Danlos doesn’t help any. I would jump on the chance to be in a trial of this type of help.

  281. I’m sure there are thousands of us who would love to experience a normal life without shoulder pain. Between more testing and government approval I don’t see this happening for years. I myself am trying one last thing in hopes it will help. Burning the nerves that are causing the pain. Fingers crossed it will help but after several surgeries it’s hard to be hopeful.

  285. Hello, is this procedure available yet in the US? Thank you for the research and hard work. Keep working on this. We need it. I am 65 and I was injured in 2014 almost 10 years ago. I am always in pain and keep a pillow under my right arm in my armpit. I have been so lost and not the same person. It ruined my life. I am active and very young 65 yr old female. Partial shoulder replacement. I would not do the RSA because of the complications and I was warned by a doctor not to do it. I hope someday there will be something available to the masses.
